Join Our Community Health Network

Quality care begins with linking our members to the right providers. That's why we contract with community-based providers like yourself to become part of our managed care network. This allows us to grow together and better serve our patients by providing specialty services and more primary care physician options.

Note: To join Omnicare's network, you must be enrolled as a California Medi-Cal and Medicare provider and/or facility and have an active California Medi-Cal and Medicare ID number.

Step One: Submit Your Required Documents

Please ensure the Contract Request Form is typed out. Partial/incomplete submissions will not be considered. Please allow at least 60 business days for review of complete submissions. Omnicare will respond to your request by email once a decision is made.

Note: Acceptance into Omnicare's network is not guaranteed. All requests are evaluated based on network needs.

Step Two: Connect With a Provider Network Administrator

If your request to participate in Omnicare's network is accepted, a Provider Network Administrator will reach out to you to answer any questions and provide you with additional required documents for you to complete. Upon receipt of all fully completed documents, your credentialing process will begin.

Step Three: Complete the Credentialing Process

Upon receipt of complete copies of all required documents, Omnicare's credentialing department will credential all providers and/or facilities identified in your documentation. The credentialing process may take up to 90 - 120 calendar days to complete. Omnicare's Credentialing department will notify you once credentialing is complete.

If you want to find out your credentialing status, please contact your assigned Provider Network Administrator for further assistance. 

Note: To facilitate and reduce delays during the credentialing process, we recommend:
 

  • Registering with CAQH and providing access

  • Completing/updating CAQH information for all physicians

  • Properly completing and signing all required documentation

  • Promptly correcting any errors found in your CAQH information during the credentialing process, as needed

Step Four: Complete the In-Service Training

Once you are credentialed, the Provider Network Administrator will provide you with a signed copy of your Agreement and reach out to you to complete the provider in-service.

Already a participating provider?

If you are already a participating provider and/or facility with Omnicare and need to notify us of updates or changes to your office or provider/facility information (i.e. terminations, address or phone number change/update, additional providers or locations to be added, TIN change), please contact your assigned Provider Network Administrator for assistance or email contracting@alturamso.com. You may validate if your request was completed by checking Omnicare’s Online Provider Portal.

Omnicare generally responds to requests by email within at least 60 business days following receipt of complete submissions. If your submitted request is less than 60 business days old, please wait to be contacted.
